¿Por qué es necesario este concepto?

El secuenciamiento de nueva generación (NGS) puede identificar una gran cantidad de variantes en un individuo. Sin embargo, el hecho de que se haya detectado una variante no permite concluir por sí solo que dicha variante explique los síntomas o la enfermedad actuales.

La pregunta central de la interpretación clínica es:

¿Dispone este cambio genético de evidencia suficiente para estar asociado con el fenotipo observado en esta persona?

El genotipo se refiere al estado de las variantes y los genes, mientras que el fenotipo abarca las características observables, como síntomas, signos, resultados de pruebas y patrones de aparición de la enfermedad. La correlación genotipo-fenotipo no es simplemente una tabla que empareja ambos elementos, sino un proceso que integra múltiples evidencias para evaluar la fuerza y la incertidumbre de dicha relación.

Los resultados del NGS pueden incluir numerosas variantes candidatas, dependiendo del objetivo del análisis y su calidad. Solo algunas de estas tienen probabilidad de estar relacionadas con la enfermedad actual, y esa posibilidad debe responder a las siguientes preguntas:

  • ¿Qué relación funcional tiene esta variante con qué gen?
  • ¿Con qué enfermedades o fenotipos está asociado dicho gen?
  • ¿Coincide el fenotipo del paciente con el espectro conocido de dicha enfermedad?
  • ¿Es coherente el patrón de herencia y la transmisión familiar?
  • ¿Apoyan otros datos independientes la patogenicidad?

In this flow, variant pathogenicity, gene–disease validity, patient phenotype match, and clinical actionability constitute separate layers of judgment. Support from a preceding step does not automatically confirm the next one.

How to Read Evidence Together

Quality of Phenotype

Information stating only that "a symptom is present" may be insufficient. The strength of the connection between genotype and phenotype varies depending on when the symptom started, which tests confirmed it, and whether there is a characteristic combination. If the phenotypic record is ambiguous, interpretation can become unstable even for the same variant.

Mode of Inheritance and Family Information

Modes of inheritance such as autosomal dominant/recessive or X-linked alter the scope of variant interpretation. How a variant is transmitted in family testing can also be an important clue; however, when family size is small or information is incomplete, it is difficult to use as a sole basis for conclusion.

Population Frequency

Considering disease prevalence and mode of inheritance, how common a variant is in the general population affects interpretation. However, rarity alone does not prove pathogenicity.

Functional, Segregation, and Computational Evidence

Experimental functional data, within-family co-segregation, computational predictions, and existing literature are different types of evidence. Since each has different strengths and conditions of application, one must distinguish which evidence supports which question rather than lumping them together under "there is much evidence."

A Small Thought Experiment

Suppose a rare variant in gene GENE-X was found in a patient's NGS. The following three scenarios lead to different conclusions.

Common Points of Confusion

Does the presence of a variant mean it causes the disease?

No. A variant is an observed genetic difference, while pathogenicity is a classification judgment based on synthesizing multiple lines of evidence. There is an interpretation step between the presence of a variant and its role as a disease cause.

If the phenotype does not match, is the genotype wrong?

Not necessarily. The phenotypic record may be incomplete, or expression may vary due to penetrance, expressivity, age dependence, or genetic background. However, mismatches should not be hidden but recorded as uncertainty in interpretation.

Can ACMG/AMP criteria alone allow all genes to be interpreted in the same way?
