Novartis (NVS) has commenced patient enrollment and dosing in the Phase 1/2 trial (CYStem study, NCT06910813) of its gene therapy candidate, 'DFT383,' for Nephropathic Cystinosis. Innovative Treatment Mechanism Based on Autologous Hematopoietic Stem Cells
DFT383 utilizes a mechanism that involves isolating the patient's autologous CD34+ hematopoietic stem and progenitor cells (HSPCs) and transducing them with a lentiviral vector to express normal CTNS genes, which are then re-infused into the patient. This technology aims to restore the normal production of cystinosin protein within the body, thereby fundamentally preventing the accumulation of cystine crystals in cells. While existing treatments only provide symptomatic relief, DFT383 represents a curative approach to address the underlying cause of the disease.
Overcoming Limitations of Existing Treatments and Promising Clinical Data
The current standard of care (SOC) for Nephropathic Cystinosis relies on cysteamine (brand name Procysbi) to inhibit cystine accumulation. However, Procysbi requires lifelong administration twice daily and can cause severe gastrointestinal side effects and a distinct body odor, leading to poor patient compliance. In contrast, preliminary results from a previous Phase 1/2 trial of DFT383 showed that all six adult patients who received the therapy experienced a sustained reduction in leukocyte cystine levels below baseline, with some patients able to discontinue oral cysteamine for up to 36 months.

Recognizing the potential of this therapy, Novartis acquired the entire program from AVROBIO in May 2023 for an upfront payment of $87.5 million. This all-cash deal, without milestones or royalties, demonstrates Novartis's commitment to maximizing its first-mover advantage, even if it means taking on some risk. The global market for Nephropathic Cystinosis treatment is estimated at $198.8 million in 2024 and is projected to grow to $465 million by 2033, making it a niche market with high growth potential. Future Clinical Milestones and Long-Term Prospects
This trial began on June 2, 2025, and is on track to complete the primary phase by May 2031. Given that the gene is being introduced to young, growing pediatric patients, a long-term extension phase will be conducted to track safety and durability for up to 15 years. If Novartis can demonstrate complete safety in pediatric patients, it will not only provide innovative improvements in quality of life for patients who have suffered for a lifetime but also maximize the value of its rare disease portfolio.
