- Emphysema is a form of chronic obstructive pulmonary disease (COPD), a chronic lung disease in which the alveolar walls are damaged, making the exchange of oxygen and carbon dioxide difficult.
- The most common related factor is long-term exposure to tobacco smoke; secondhand smoke, inhaled irritants such as air pollution, chemicals, and dust, and rarely, alpha-1 antitrypsin deficiency may also be involved.
- When the alveolar walls are destroyed, the alveoli lose their elasticity and merge into larger, fewer spaces, thereby reducing the function of exhaling and taking in oxygen.
- Representative patterns include dyspnea (shortness of breath) during activity, chronic cough, sputum, wheezing, whistling-like breath sounds, and chest tightness.
- Risk varies according to the amount and duration of smoking, exposure to other inhaled irritants, age, genetic factors, and family history; even within the same COPD category, the degree of emphysema and symptoms can vary per individual.
- In the early stages, symptoms may be absent or mild, but they generally worsen over time, and in severe cases, recurrent respiratory infections, weight loss, muscle weakness, and leg edema may appear.
- Diagnosis is made by confirming symptoms, smoking history, exposure history, and family history, and then comprehensively integrating pulmonary function tests, chest X-ray or CT, and blood tests as necessary.
- Do not conclude severity or the cause of symptoms based solely on imaging findings; they must be interpreted alongside pulmonary function, symptoms, and history of exacerbation, and comorbid conditions such as chronic bronchitis must also be considered.
- Management centers on smoking cessation and avoidance of irritants, while simultaneously addressing symptom control, maintenance of physical activity, prevention of exacerbations and infections, and assessment of comorbidities.
- Although there is currently no treatment to completely reverse emphysema, the goal of treatment is to improve symptoms and function and to reduce disease progression and complications.
